1. Irregular or Absent Periods
A normal menstrual cycle ranges from 21 to 35 days. If your cycles are consistently shorter, longer, or absent, it may indicate hormonal imbalances such as PCOS, thyroid disorders, or premature ovarian insufficiency — all of which can affect fertility.
What to do: Track your cycle for 3 months. If cycles vary by more than 7 days consistently, see a gynaecologist.
2. Painful Periods (Dysmenorrhoea)
Moderate period pain is normal. But severe cramps that prevent normal activity, or pain that worsens over time, may be a sign of endometriosis — one of the leading causes of infertility. Endometriosis affects 10% of women of reproductive age and is frequently under-diagnosed.
3. Heavy or Very Light Periods
Extremely heavy periods (soaking more than one pad/hour for several hours) may indicate fibroids, adenomyosis, or hormonal issues. Very light periods or spotting only may suggest inadequate endometrial lining — which can impair embryo implantation.
4. Pain During Intercourse

Signs of androgen (male hormone) excess include:

9. Chronic Pelvic Pain
Persistent pain in the lower abdomen (outside of periods) may indicate pelvic inflammatory disease, ovarian cysts, fibroids, or endometriosis — all potential causes of fertility problems.
10. Previous Sexually Transmitted Infections (STIs)
Chlamydia and gonorrhoea, even when treated, can cause tubal damage that is often silent and symptom-free for years. Damaged tubes are a leading cause of infertility and ectopic pregnancy. A hysterosalpingogram (HSG) test can check tubal patency.
