What is the ERA Test?
ERA stands for Endometrial Receptivity Assay. It is a genetic test of your endometrial (uterine lining) biopsy that identifies your personal Window of Implantation (WOI) โ the specific hours when your uterus is most receptive to an embryo.
Most women have their WOI at the standard time (day LH+7 or P+5 in a programmed cycle). But approximately 20โ30% of women โ particularly those with recurrent IVF failure โ have a displaced WOI. Their embryo is transferred at the "standard" time, but their uterus is not yet ready (pre-receptive) or has already closed (post-receptive).
How Is the ERA Test Done?
- You undergo a mock embryo transfer cycle โ oestrogen and progesterone are given exactly as in a real FET cycle
- On the day you would normally have your embryo transfer, an endometrial biopsy is taken (a small tissue sample from the uterine lining)
- The sample is sent to a specialised lab (Igenomix/equivalent) and the genetic expression of 248 genes is analysed
- The report classifies your endometrium as: Receptive / Pre-receptive / Post-receptive โ and gives a personalised transfer time
- Your next FET is scheduled at YOUR personalised window, not the standard time
Who Actually Needs the ERA Test?
The ERA test is NOT recommended routinely for all IVF patients. It is specifically indicated for:
- Recurrent implantation failure โ 2 or more failed transfers of good-quality embryos (especially blastocysts)
- Repeated IVF failure with unexplained cause โ all other factors (sperm, embryo quality, uterus anatomy) have been checked
- Thin endometrium with repeated failure
- Frozen embryo transfer patients who failed despite good embryo quality and normal uterine cavity
If this is your first IVF cycle and you have good embryo quality and no history of failure, you do NOT need the ERA test. The standard protocol is appropriate.
Does the ERA Test Improve Success Rates?
The evidence is nuanced. In the largest randomised trial (IGENOMICS ERA, 2021), ERA-guided transfer did not improve success rates in first-time IVF patients. But in patients with recurrent implantation failure, ERA-guided transfer showed a meaningful improvement in clinical pregnancy and live birth rates (approximately 15โ20% improvement in this subgroup).
