What Is Antral Follicle Count (AFC)?
The AFC is the number of small (2–10mm) fluid-filled follicles visible in both ovaries on a Day 2–5 ultrasound scan. Each antral follicle has the potential to grow and produce a mature egg during an IVF stimulation cycle. AFC is one of the best real-time predictors of how many eggs an IVF cycle will produce.

AFC 2 or 3 — Is IVF Still Possible?

DuoStim — Getting Two Retrievals in One Month
For AFC 2–4 patients who want to accumulate embryos before transfer: DuoStim (Dual Stimulation) allows two egg retrievals in one menstrual cycle — once in the follicular phase and once in the luteal phase. This doubles the eggs collected per month and is particularly valuable when AFC is very low.
Embryo Banking Strategy for Low AFC
With AFC 2–3, you may get 1 egg per cycle. The strategy: freeze any resulting embryos from multiple cycles, then transfer the best one(s). This banking approach — typically over 2–3 cycles — gives a cumulative success rate of 40–60% even when individual cycle yield is low.
Can AFC Improve?
AFC reflects the follicles your ovaries happen to have at that point in time. It can vary slightly cycle to cycle but does not significantly "improve." DHEA priming does not create new follicles — it makes existing follicles respond better to FSH. An AFC of 2 may yield 1 egg today and 3 eggs next month simply because of natural variation.
