What Is a Low Sperm Count? The Numbers Explained
WHO 2021 reference values: Normal = โฅ16 million sperm per mL (โฅ39 million per ejaculate). Below normal is called oligospermia. Below 5 million/mL is severe oligospermia. Below 1 million/mL is extremely severe oligospermia (cryptozoospermia).
But here is the key point: you need exactly ONE sperm to fertilise one egg. With ICSI (Intracytoplasmic Sperm Injection), a single viable sperm is all that is needed per egg. Even sperm count at 1 million/mL gives 1 million sperm to work with.
Sperm Count 1 Million/mL โ Can ICSI Work?

Sperm Count 5 Million/mL โ ICSI vs Natural Conception
At 5 million/mL, natural conception is very unlikely (most sperm do not reach the egg at this count). IUI has a 10โ15% success rate per cycle but requires at least 5 million motile sperm after washing. ICSI-IVF bypasses this entirely and gives 45โ55% success per transfer regardless of sperm count โ the embryologist picks the single best sperm for each egg.
Improving Sperm Count Before ICSI
For men with 1โ5 million/mL: a 90-day protocol of Antioxidants (CoQ10 300mg, Vitamin C 1g, Vitamin E 400 IU, Selenium 200mcg) + Folic acid 5mg has been shown to improve count and DNA fragmentation. Combined with treating any varicocele if present, this can sometimes shift sperm count above 5 million before the IVF cycle โ increasing the choice of sperm for ICSI and potentially reducing DNA fragmentation rates.
TESA โ When Ejaculated Sperm Are Too Few
If sperm count falls below the threshold needed even for ICSI โ or if DNA fragmentation is very high in ejaculated sperm โ TESA (Testicular Sperm Aspiration) retrieves sperm directly from the testis. Testicular sperm often have lower DNA fragmentation than ejaculated sperm, which can improve fertilisation rates in severe oligospermia.
