What Is Beta HCG and When Is It Tested After IVF?

Beta HCG (beta human chorionic gonadotropin) is the hormone produced by the embryo as soon as it implants in the uterine lining. In IVF, the blood test is done exactly 14 days after a Day-3 embryo transfer, or 14 days after a Day-5 (blastocyst) transfer. This is called the "14dp3dt" or "14dp5dt" test.

A positive beta HCG confirms implantation. How positive depends on the number โ€” but a rising HCG that doubles every 48โ€“72 hours is more important than the first number alone.

HCG 10 or 20 After Transfer โ€” Is It a Pregnancy?

HCG 10โ€“20 at 14dp5dt is a very early positive โ€” sometimes called a "chemical pregnancy." At this level:

  • Continue all progesterone/estrogen support as prescribed
  • Repeat blood test in exactly 48 hours
  • If HCG rises to 20โ€“40 (doubling), this is reassuring โ€” continue support and retest again at 96h
  • If HCG stays flat or drops below 10, this represents an early biochemical pregnancy loss โ€” the embryo implanted briefly but did not develop

Do NOT stop progesterone based on a first low HCG result. The second result is what matters.

HCG Doubling Time โ€” What Is Normal?

Healthy early pregnancy HCG doubles every 48โ€“72 hours until about week 8โ€“10. For example:

  • Day 14: HCG = 50 โ†’ Day 16: HCG = 100โ€“150 = Good doubling
  • Day 14: HCG = 50 โ†’ Day 16: HCG = 55 = Concerning (less than 50% rise in 48h)
  • Day 14: HCG = 50 โ†’ Day 16: HCG = 200+ = Excellent rise (possible twins)

Beta HCG After Day-3 Transfer vs Day-5 Transfer

For a Day-3 transfer: the 14-day beta test is done 14 days post-transfer (at approximately 17dp3dt in embryo age). Expected values are similar โ€” a positive is typically >25 mIU/mL. A Day-3 embryo has had 3 extra days to implant compared to a blastocyst, but HCG production starts from the same point (implantation).

Low Beta HCG โ€” What Does It Mean for a Next Cycle?

A biochemical pregnancy (HCG positive then drops) means the embryo implanted but did not sustain. This is actually informative โ€” it confirms the embryo reached the uterus and the uterine lining was receptive enough for implantation.
