ICSI అంటే ఏంటి? Regular IVF కంటే ఎలా different?
Regular IVF లో sperm మరియు egg ని lab dish లో కలిపి natural fertilisation జరుగుతుందని wait చేస్తాం. ICSI (Intracytoplasmic Sperm Injection) లో — ఒక్క healthy sperm ని select చేసి, directly egg లో inject చేస్తాం. Sperm count చాలా తక్కువగా ఉన్నా, motility poor అయినా — ICSI work చేస్తుంది. Fertilisation rate 70–80%.
IVF egg retrieval తర్వాత — embryologist ఒక్క healthy sperm ని select చేస్తారు. Fine glass needle తో egg లో directly inject చేస్తారు. 2–3 hours లో done. Fertilised eggs 5 రోజులు culture చేస్తాం — blastocyst stage వరకు.
Low sperm count (< 5 million). Poor motility (< 20%). High abnormal morphology. Previous IVF లో fertilisation fail అయితే. TESA sperm (testis నుండి). Anti-sperm antibodies. ఇవి ఏదైనా ఉంటే — ICSI standard choice.
Standard IVF insemination: 60–70% fertilisation. ICSI: 70–80%. అంటే — 10 mature eggs ఉంటే, ICSI తో 7–8 eggs fertilise అవుతాయి. More embryos = more chances = better outcomes.
PICSI లో hyaluronic acid binding తో genetically mature sperm select చేస్తాం. High sperm DNA fragmentation ఉన్న patients కి PICSI better results ఇస్తుంది. Miscarriage risk తగ్గుతుంది.
Semen లో sperm లేదు (azoospermia) అయినా — TESA procedure తో testis నుండి sperm retrieve చేసి ICSI చేయవచ్చు. 30–50% non-obstructive cases లో sperm దొరుకుతుంది. Local anaesthesia, 20–30 minutes.
